A good reminder to bring kids in for a comprehensive eye exam even if they are not complaining about their vision! I see a ton of kids that think they see great, but actually don’t, and then you have cases like this where there’s an eye health concern that would have gone undetected if not for an eye exam (and a great photo- shoutout to our technician, Candace).

Something that surprises a lot of people: conditio Something that surprises a lot of people: conditions affecting the brain (like multiple sclerosis, tumors, aneurysms, etc) can affect the eyes, and are sometime even diagnosed as a result of abnormal findings in a routine eye exam. These photos show a right and left optic nerve, both of which are swollen. I know some of my patients found me on social medi I know some of my patients found me on social media specifically to see my road trip pics, so here are a few! For those that don’t know, I made the long overdue decision to quit my job earlier this year. I took advantage of the opportunity to take some time off; off from working, off from accomplishing, off from being productive. We as a society tend to glorify hustle, but what I think we need more of is stillness. I’m thankful for the opportunity to pause, reflect, and reset. 

I’m looking forward to getting back to patient care next week! For my patients, I have created (read: my brother created for me) a simple webpage that I’ll update with any changes to my work locations. There’s also a place to sign up for email updates if/when things change! Link in profile.

There’s an eye museum in San Francisco! 😯 I s There’s an eye museum in San Francisco! 😯 I stumbled upon it while walking around the city and obviously had to check it out. It’s free, run by the @aaoeye , and definitely worth visiting (esp if you’re an eye nerd). Lots of interesting timelines and artifacts, as well as some interactive activities for little and big kids.

A couple of fun facts I learned:

👁 Mila Kunis reportedly struggled with a chronic iritis (inflammation of the inner eye) in her twenties. 

👁 WWII airplane cockpit canopies, seen in the last photo, were made of PMMA, the plastic material that inspired Sir Harold Ridley to design the first intraocular lens implant (because the splinters from it did not cause inflammatory rejection).

It made me think about how much a simple, unexpected act of kindness can brighten a day, and I was challenged to remember the last time I did something like that for someone else. So I’m passing along the challenge to y’all: find a small way to make someone’s day this week! We all need it.
A lot of people think that having good vision mean A lot of people think that having good vision means you don't need an eye exam. While having good vision may mean that you don't need glasses or contact lenses, it does not necessarily mean your eyes are in good health! All of the conditions listed here have walked into my exam room over the past couple of years, and many of them had no visual complaints. 

👁 The eyes are essentially an extension of the brain, so a thorough eye exam can help diagnose certain neurological issues.

👁 The eyes are the only place where you can directly and non-invasively examine the blood vessels of the body, so a thorough eye exam can also help diagnose vascular issues. 

Over the next few weeks, I'll share some interesting cases from clinic that involve diagnoses of multiple sclerosis, a brain aneurysm, sarcoidosis, and dangerously high blood pressure, all through the findings of an eye exam!

Can the medications you take affect your eyes? Y Can the medications you take affect your eyes? 

Yes! This case illustrates just one of the many ocular manifestation/effects of systemic medications. 

Amiodarone is a drug used to treat irregular heartbeat. Amiodarone can have side effects on many organs, including the eye. Vortex keratopathy (aka cornea verticillata aka whorl keratopathy), seen here, is the most common ocular change caused by amiodarone. It is believed that the drug leads to an accumulation of intracellular phospholipids, which results in this whorl-like pattern of deposition in the outer layer of the cornea. 

Though most with this finding are asymptomatic, some may report light sensitivity or haloes around lights.

There are other medications and conditions associated with this finding, such as Fabry’s disease and chloroquine use.

Myopia is more than just blurry distance vision! H Myopia is more than just blurry distance vision! Higher amounts of myopia have been associated with increased risk of certain eye diseases. 

Compared to a child with no prescription, a -2.00 D myope has a 2 times higher risk of myopic macular degeneration (MMD) and cataracts, and a 3 times higher risk of retinal detachment (RD). Jump to a -5.00 and the risk increases substantially: 41x higher risk of MMD, 9x higher risk of RD, and 3x higher risk of developing cataracts. The risk increases with every point of myopia, so it’s super important for parents to consider myopia management therapies to slow their children’s myopia progression!

Fortunately, it was just a superficial injury and a burst blood vessel. But we had a good laugh imagining what a chicken mutiny would look like (basically the chicken version of Jurassic Park).

“My eyes feel fine and my vision is great- I don “My eyes feel fine and my vision is great- I don’t need a full eye exam.” Here’s one of many cases to help dispel this popular myth. 

These photos show a patient’s lens through a dilated pupil (same eye, different illumination). The white deposits on the outer part of the anterior lens capsule point to a systemic condition called pseudoexfoliation syndrome, or PXF. The fluffy white fibrillar material deposits on many structures in the eye (and elsewhere in the body), which can cause problems; PXF is the most frequent cause of secondary glaucoma, as well as a major risk factor for complications during cataract surgery. 

What are the symptoms? *There are no symptoms.* 

Comprehensive eye exams are important for everyone, regardless of how good your vision is or how great your eyes feel!

The earlier the onset of myopia (nearsightedness), The earlier the onset of myopia (nearsightedness), the greater the risk of progression to higher levels of myopia. The higher the myopia, the greater the risk of sight threatening complications.

BUT we now have proven ways to slow the progression of myopia! The earlier we start intervention, the better, so don’t wait for your kids to report problems to bring them in to the optometrist!
